NHIF, outpatient facilities and the doctors available..

I had to visit the Wellcare Medical centre last tuesday (12th October 2021), for a problem which was getting worse by the day. 

Fortunately, I didn't have to wait long to see a doctor after I had gone through the initial process at the reception after I presented my NHIF card.

The doctor is a GP (I think), who asked to see my health issue and when I showed it to him, he 'jumped' away and was very upset telling me that what I had was very contagious and the name he gave me was something I had heard about, but did not know much else. Anyway, I was given some medicines to be applied to the site and told to even get my place fumigated!

The medicines, after more than 3 days of usage, did not make the slightest difference and the problem got even worse! Also, as soon as I got home from the clinic, I went online to find out more about the problem I was supposed to be suffering from and found that I could only have 'caught' it from another human in close contact or if I'd shared my personal stuff with another who suffered from the same problem.

Now, I live on my own and hardly get any 'chance' to be in that kind of close contact with anybody and especially during these covid times, it is better to keep as far away as possible from another person. Covid-19 has made all of us very anti-social where we don't shake hands nor hug another however close the person might be to one.

Therefore, I could not imagine how I might have contracted this stupid disease. Also, even I, as a lay person, could see that the pictures and description did not match with what I had. So, how did this doctor jump, to the conclusion that he did? 

Therefore, I decided to see a dermatologist at my own cost and found out that it was a fungal and non-contagious problem which was easily dealt with whose effect I could see within 24 hours! 
 
Therefore, my contention is that what kind of doctors are we producing? Who don't make the effort and take the time to really find out more about the patients they are treating and their problems. Because, if this doctor had taken the time and made the effort, he would have found out that considering my circumstances, it was not very likely for me to have picked up a contagious disease and then made further efforts to find out what it really was. And if he couldn't understand, then he could have referred me to a dermatologist.

So, is the reason he behaved the way he did, due to his limited knowledge and/or because NHIF has this agreement with these outpatient clinics to make sure that they are not charged a huge amount, or what? I am saying this because in the past, I've been given wrong medicines, have had to pay extra for some further treatment, etc.

What happens with people who are NHIF members who do not have access to all the info that I had to find the truth and/or don't bother following up on such laxity? Maybe, they could even die due to being wrongly diagnosed!

Can all the people concerned, please, get their act together and make sure that NHIF members really DO get quality healthcare at their accredited facilities and that the patients who go there are not just suffering from 'simple' everyday diseases and investigate properly before prescribing those medicines?!

Being a member of NHIF... BEWARE!!!

I have just spent most of the morning and a part of my lunchtime trying to untangle the things that NHIF DOES NOT tell its subscribers!!!

It does NOT tell us that all those NHIF accredited places are NOT for ALL subscribers of NHIF. They tell all Kenyans to subscribe to them even with as little as they can afford e.g. 500/- per month which will cover their out-patients bills, too.

Ah, but there's a catch as I found out today - that not all facilities that advertise at their entrances that they are NHIF accredited are really so. Only SOME of them will agree to see you despite you paying their monthly subscription. The reason you'll be told is that ONLY civil servants and the police are going to be seen there. Not the rest of us common Kenyans!

In case, you have downloaded their 'application', there's no end to the frustration you'll encounter to get information, though it claims that it is there for our convenience. Believe me, it is anything but that.

This 'app' only tells you that you're paid up or not, but not how long you are paid up, for, i.e. there's no way to find out up to what month you have paid. You send them a message from the same app and you'll NEVER get a reply!

Next, since they tell you that you are also covered for out-patient services, you have to choose from a long line of clinics and hospitals that are the most convenient for you. Well, there's a catch there. 

At no place does NHIF inform you that you can register yourself only at certain clinics and hospitals - not all of those that are listed there. Unfortunately, since they don't and I did not know that Premier Hospital in Nyali was not the one for me because it did not tell me so when I was registering there from their app, I went there today when I was in a lot of pain.

Soon after I presented my ID and NHIF card to them, I was told very politely by one of the girls working there that they only entertained and treated civil servants and cops NOT a commoner like me!

So now, I had to find another place that would accept my card, but since their app does not tell us which those are, I, and they (the two girls at Premier Hospital who were helping me out), were at a loss. They even tried calling NHIF and told them about my predicament, but didn't get any proper answer. In fact, they told the girls that I should go to their town offices to get sorted out!

Do these unfeeling people have any sense? How is somebody who is a senior citizen and not mobile, supposed to do all that? And also when I was in pain?

Therefore, I then chose to go to another clinic, where I used to go, and they had my record and which, the NHIF app told us accepted my card. I had no way of knowing if they really would.

Well, when I got there, they confirmed that they now accepted my type of NHIF card though previously they hadn't and I could now register with them. It was a relief, but again there was a catch. When the receptionist tried to register me there, it took her quite a while, but she managed it and I also got a confirmation that I was now registered there. Only problem was that I would ONLY BE ABLE TO USE MY CARD FROM 1ST JAN 2021!!!! 

Now, through no fault of mine I had to go through this run-around and that it is the fault of NHIF that I had registered myself at the 'wrong' place. So my question to this questionable outfit is - why don't they specify at their accredited facilities  that they list on their app as to which are available to the general Kenyan public and which are for the 'special people', only?! 

I would appreciate an answer as soon as possible.

NHIF payments..

I went to a clinic today where they accept our NHIF cards. After I gave my card to the receptionist, she checked if my payment was up to date.

After a few moments, she came back to me and very politely told me to please update my payments on it as it showed a very low balance!

I informed her that my payment to NHIF was fully paid up to at least April 2017. She insisted that the card did not show this and that to get in touch with NHIF immediately and inform them of this fact.

This I did as soon as I got back home. One of their numbers was not being answered, so I tried another (all of them in Nairobi as the Mombasa ones are NEVER answered!). It was answered quite fast, but, after I told the receptionist about my problem, she put me on hold for over 2 minutes and never got back to me, so I hung up.

I tried the same number after a few minutes and again it was answered immediately and the person at the other end asked me my problem which I related. She then asked for my ID number and mobile number both of which I supplied. I was told to hang up and wait for somebody to call me on the number I had given her and would not share any more details. All very mysterious. 

The above calls and conversation took place at about 3.00 pm. It is now past 5.30 pm, but nobody has called me from NHIF and I do not believe that they will.

Lots of questions started swirling in my head immediately that the clinic had told me about my NHIF card showing some very strange amount on it.

1. I have never used the card since I got it early last year, so how come it did not show ALL of the amount I've been paying them all this time?

2. What is it supposed to show anyway?

3. Since they have not been updating my card efficiently and keeping track of my payments, what happens if and when, I am so ill that I cannot show the clinic or the hospital where I am admitted the proof of payment via Mpesa on my phone? What will the medical institution do when my card does not show all these payments?

4. Why, when I  called NHIF Nairobi regarding my card update, did the receptionist behave in the way she did?

5. Why haven't I still heard from them, yet? And will I ever do....?! 

6. So now, what happens?
All the above thoughts are still going through my mind and which also make me very angry. Angry at our so-called institutions whom we pay through our collective noses, but get NOTHING in return. Total unaccountability and irresponsible attitude.

Yet, just this morning we read that the poor medical personnel who have been trying to get justice, have been trashed and told arrogantly to return to work or else become jobless.

Is it any wonder that we are seriously suffering a braindrain in this country? Everybody who can and have the opportunity are leaving. It's pathetic.

How come the citizens who are paying our useless politicians who do the least amount of work, but have awarded themselves obscene amounts of remuneration, were told by their employers (us Kenyans), that we will stop paying them a cent till they do some honest amount of work on OUR BEHALF, for once?!

Let us see, if they like it? Maybe, then they will understand the Medics' plight.

All this is connected with our NHIF payments. Where is the money going?

I  need answers, pronto!

DIY is so easy and its costs are unbelievably negligible.
Mosquito trap (Dengue prevention)
Its just a mix of water, brown sugar and yeast.

1. Cut a plastic bottle in half, keep both parts. Can be soft drink bottle.

2. Take the lower portion of the bottle. Dissolve the brown sugar in hot water.
Let it cool down to ~70 deg F (room temperature).
3.
Add the yeast. Carbon dioxide will form. (This will attract the mosquitoes.)
4.
Cover the bottle with a dark wrap and insert in the top portion upside down like a funnel. Place it in a corner in your house.
5. In 2 weeks you will be surprised by the number of mosquitoes killed.

Treating Burns

  • Third-degree burns cause damage to all layers of the skin. The burned skin looks white or charred. These burns may cause little or no pain because the nerves and tissue in the skin are damaged.

How long does it take for burns to heal?

  • First-degree burns usually heal in 3 to 6 days.
  • Second-degree burns usually heal in 2 to 3 weeks.
  • Third-degree burns usually take a very long time to heal.

How are burns treated?

The treatment depends on what kind of burn you have.
See a doctor if:
  • A first- or second-degree burn covers an area larger than 2 to 3 inches in diameter.
  • The burn is on your face, over a major joint (such as the knee or shoulder), on the hands, feet or genitals.
  • The burn is a third-degree burn, which requires immediate medical attention.

First-degree burn

Soak the burn in cool water for at least 5 minutes. The cool water helps reduce swelling by pulling heat away from the burned skin.
Treat the burn with a skin care product that protects and heals skin, such as aloe vera cream or an antibiotic ointment. You can wrap a dry gauze bandage loosely around the burn. This will protect the area and keep the air off of it.
Take an over-the-counter pain reliever, such as acetaminophen (one brand name: Tylenol), ibuprofen (some brand names: Advil, Motrin) or naproxen (brand name: Aleve), to help with the pain. Ibuprofen and naproxen will also help with swelling.

Second-degree burn

Soak the burn in cool water for 15 minutes. If the burned area is small, put cool, clean, wet cloths on the burn for a few minutes every day. Then put on an antibiotic cream, or other creams or ointments prescribed by your doctor. Cover the burn with a dry nonstick dressing (for example, Telfa) held in place with gauze or tape. Check with your doctor's office to make sure you are up-to-date on tetanus shots.
Change the dressing every day. First, wash your hands with soap and water. Then gently wash the burn and put antibiotic ointment on it. If the burn area is small, a dressing may not be needed during the day. Check the burn every day for signs of infection, such as increased pain, redness, swelling or pus. If you see any of these signs, see your doctor right away. To prevent infection, avoid breaking any blisters that form.
Burned skin itches as it heals. Keep your fingernails cut short and don't scratch the burned skin. The burned area will be sensitive to sunlight for up to one year, so you should apply sunscreen to the area when you're outside.

Third-degree burn

For third-degree burns, go to the hospital right away. Don't take off any clothing that is stuck to the burn. Don't soak the burn in water or apply any ointment. If possible, raise the burned area above the level of the heart. You can cover the burn with a cool, wet sterile bandage or clean cloth until you receive medical assistance.

Is there anything I shouldn't do when treating a burn?

Do not put butter or oil on burns. Do not put ice or ice water directly on second- or third-degree burns. If blisters form over the burn, do not break them. These things can cause more damage to the skin.

What do I need to know about electrical and chemical burns?

A person who has an electrical burn (for example, from a power line) should go to the hospital right away. Electrical burns often cause serious injury to organs inside the body. This injury may not show on the skin.
A chemical burn should be flushed with large amounts of cool water. Take off any clothing or jewelry that has the chemical on it. Don't put anything on the burned area, such as antibiotic ointment. This might start a chemical reaction that could make the burn worse. You can wrap the burn with dry, sterile gauze or a clean cloth. If you don't know what to do, call 911 or your local poison control center, or see your doctor right away.

What causes burns?

An Eye Opener on Ovarian Cancer

I hope you all take the time to read this and pass it on to all you can.  Send this to the women in your life that you care about.

A few years ago, Gilda Radner died of ovarian cancer.  Her symptoms were inconclusive, and she was treated for everything under the sun until it was too late.   This blood test finally identified her illness but alas, too late.  She wrote a book to heighten awareness.  Gene Wilder is her widower.
 
KATHY'S STORY:  this is the story of Kathy West

As all of you know, I have Primary Peritoneal Cancer.  This cancer has only recently been identified as its OWN type of cancer, but it is essentially Ovarian Cancer.

Both types of cancer are diagnosed in the same way, with the "tumour marker" CA-125 BLOOD TEST, and they are treated in the same way - surgery to remove the primary tumour and then chemotherapy with Taxol and Carboplatin.

Having gone through this ordeal, I want to save others from the same fate.  That is why I am sending this message to you and hope you will print it and give it or send it via E-mail to everybody you know.

One thing I have learned is that each of us must take TOTAL responsibility for our own health care.  I thought I had done that because I always had an annual physical and PAP smear, did a monthly Self-Breast Exam, went to the dentist at least twice a year, etc.  I even insisted on a sigmoidoscopy and a bone density test last year.  When I had a total hysterectomy in 1993, I thought that I did not have to worry about getting any of the female reproductive organ cancers.

LITTLE DID I KNOW.  I don't have ovaries (and they were HEALTHY when they were removed), but I have what is essentially ovarian cancer.   Strange, isn't it?

These are just SOME of the things our Doctors never tell us: ONE out of every 55 women will get OVARIAN or PRIMARY PERITONEAL CANCER.

The "CLASSIC" symptoms are an ABDOMEN that rather SUDDENLY ENLARGES and CONSTIPATION and/or DIARRHEA.

I had these classic symptoms and went to the doctor.  Because these symptoms seemed to be "abdominal", I went to a gastroenterologist.   He ran tests that were designed to determine whether there was a bacteria infection; these tests were negative, and I was diagnosed with "Irritable Bowel Syndrome".  I guess I would have accepted this diagnosis had it not been for my enlarged abdomen.   I swear to you, it looked like I was 4-5 months pregnant!  I therefore insisted on more tests.

They took an X-ray of my abdomen; it was negative.  I was again assured that I had Irritable Bowel Syndrome and was encouraged to go on my scheduled month-long trip to Europe.   I couldn't wear any of my slacks or shorts because I couldn't get them buttoned, and I KNEW something was radically wrong.  I INSISTED on more tests, and they reluctantly) scheduled me for a CT-Scan (just to shut me up, I think).   This is what I mean by "taking charge of our own health care."

The CT-Scan showed a lot of fluid in my abdomen (NOT normal).  Needless to say, I had to cancel my trip and have FIVE POUNDS of fluid drawn off at the hospital (not a pleasant experience I assure you), but NOTHING compared to what was ahead of me.

Tests revealed cancer cells in the fluid.  Finally, finally, finally, the doctor ran a CA-125 blood test, and I was properly diagnosed.

I HAD THE CLASSIC SYMPTOMS FOR OVARIAN CANCER, AND YET THIS SIMPLE CA-125 BLOOD TEST HAD NEVER BEEN RUN ON ME, not as part of my annual physical exam and not when I was symptomatic.   This is an inexpensive and simple blood test!

PLEASE, PLEASE TELL ALL YOUR FEMALE FRIENDS AND RELATIVES TO INSIST ON A CA-125 BLOOD TEST EVERY YEAR AS PART OF THEIR ANNUAL PHYSICAL EXAMS.

Be forewarned that their doctors might try to talk them out of it, saying, "IT ISN'T NECESSARY."  Believe me, had I known then what I know now, we would have caught my cancer much earlier (before it was a stage 3 cancer).   Insist on the CA-125 BLOOD TEST; DO NOT take "NO" for an answer!

The normal range for a CA-125 BLOOD TEST is between zero and 35.  MINE WAS 754.  (That's right, 754!).  If the number is slightly above 35, you can have another done in three or six months and keep a close eye on it, just as women do when they have fibroid tumours or when men have a slightly elevated PSA test (Prostatic Specific Antigens) that helps diagnose prostate cancer.

Having the CA-125 test done annually can alert you early, and that's the goal in diagnosing any type of cancer - catching it early.

Do you know 55 women?  If so, at least one of them will have this VERY AGGRESSIVE cancer.  Please, go to your doctor and insist on a CA-125 test and have one EVERY YEAR for the rest of your life.

And forward this message to every woman you know, and tell all of your female family members and friends.  Though the median age for this cancer is 56, (and, guess what, I'm exactly 56), women as young as 22 have it.   Age is no factor.
 
A NOTE FROM AN RN:
Well, after reading this, I made some calls.  I found that the CA-125 test is an ovarian screening test equivalent to a man's PSA test prostate screen (which my husband's doctor automatically gives him in his physical each year and insurance pays for it).   I called the general practitioner's office about having the test done.  The nurse had never heard of it.  She told me that she doubted that insurance would pay for it.  So I called Prudential Insurance Co, and got the same response.  Never heard of it - it won't be covered.
 
I explained that it was the same as the PSA test they had paid for my husband for years.  After conferring with whomever they confer with, she told me that the CA-125 would be covered.
 
It is $75 in a GP's office and $125 at the GYN's.  This is a screening test that should be required just like a PAP smear (a PAP smear cannot detect problems with your ovaries).  And you must insist that your insurance company pay for it.
 
Gene Wilder and Pierce Brosnan (his wife had it, too) are lobbying for women's health issues, saying that this test should be required in our physicals, just like the PAP and the mammogram.   PLEASE TAKE A MOMENT TO SEND THIS OUT TO ALL THOSE YOU CAN.  BE IT MALE OR FEMALE, IT SHOULD NOT MATTER, AS THEY CAN FORWARD IT ALSO TO THOSE LOVED ONES THEY KNOW.

Anxious or Depressed: Are You in Need of Islam?

The recent tragedy of death of 20 elementary school children in Newtown, Connecticut, has brought to focus the bitter reality that almost 20-25% of the general population suffers from anxiety and depression.
According to a BBC survey from 2008, almost one billion people world wide are suffering from anxiety and depression.
If you are one of the billion and are anxious and depressed, you may need anti-anxiety medication and anti-depressants and professional advice, but many or most of you only need a better understanding of yourself and your psychologists.  There are a billion of you out there and I cannot be presumptuous enough to be your physician, your psychologist, your psychiatrist or your counselor.  But, I can be your librarian and your spiritual guide and point you the information that you may badly need.
A famous, almost legendary mystic poet from Sub-continent India, Bulleh Shah  (1680–1757) a Punjabi Sufi poet, humanist and philosopher wrote a very insightful couplet and the actual text can be read in the Muslim Times.  To listen to this couplet and another one by him, stressing the need of self analysis, in Punjabi:Bulleh Shah – Self analysis.
He says that you have read scores of books to become a scholar but have never read yourself.  You run to the temple and the mosque daily but have never entered your own soul!
As you try to understand yourself, you are confronted not only with your own rationalizations but also with the modern science of psychology.  It helps you in some ways but at the same time sets up road blocks in your more complete understanding.  If you have sought professional help then as your psychologist begins to analyze you, you have no choice but to analyze him or her, if you are not to surrendered your peace of mind, to forces that are beyond your control, to the alchemy of modern day psychology, a 100 billion dollar industry.
Sigmund Freud (1856-1939) and his colleagues defined man as a purely "Psychological man" driven by his past experiences and memories as opposed to "Religious man" who has a constant attraction and a drive towards his Creator and Protector, the God of Monotheism. Freud theorized that personality is developed by person's childhood experiences. He was not vague about his claims for atheism. He actually predicted that as the masses of people become more educated, they would 'turn away' from the 'fairy tales of religion.' Among other details, we will examine how his views were shaped by the anti-Semitism of his time.
Carl Jung, a contemporary of Freud took an exception to Freud's views, he wrote, "Freud has unfortunately overlooked the fact that man has never yet been able single-handed to hold his own against the powers of darkness — that is, of the unconscious. Man has always stood in need of the spiritual help which each individual's own religion held out to him."
Totally on the opposite pole of Freud in matter of religion, Carl Jung explained at length, in the chapter, Psychotherapists or clergy of his book, Modern Man in Search of a Soul:
During the past thirty years, people from all the civilized countries of the earth have consulted me. I have treated many hundreds of patients. … Among all my patients in the second half of life — to say, over thirty-five — there has not been one whose problem in the last resort was not that of finding a religious outlook on life. It is safe to say that every one of them fell ill because he had lost that which the living religions of every age have given to their followers, and none of them has been really healed who did not regain his religious outlook.
Sigmund Freud wrote in a letter to Carl Jung, dated January 17, 1909, "The more the fruits of knowledge become accessible to men, the more widespread is the decline of religious belief." In his essay on war & death, he wrote, "Religion is an illusion, and it derives its strength from its readiness to fit in with our instinctual wishful impulses." Both Sigmund Freud and Carl Jung were psychologists of great repute. So, are we to believe in "Psychological man" of Freud or the "Religious man" of Carl Jung?
In the last century it seems that Carl Jung lost to Sigmund Freud as it became increasingly apparent that Christianity is often a religion of make belief, like Santa Claus, Easter Bunny and the nativity scene, which according to the Pope Benedicts XVI's own admission, in a recent book about Jesus, may peace be on him, reveals many a myths like inclusion of animals in the scene.  The Pope confessed that the animals in the nativity scene were a much later interpolation and not based in history or reality.
If you are anxious or depressed, in the 21st century, you need to examine the "Psychological man" of Freud or the "Religious man" of Carl Jung, in a new light, in the light of Islam, as understood by the Ahmadiyya Muslim Community.
A belief in Santa Claus, tooth fairy, Easter bunny, little purple man, Triune God of Christianity, who does not exist, Harry Potter,  Superman or Spider-man can serve no useful purpose.  According to the Holy Quran:
Unto Him (Allah) is the true prayer. And those on whom they call besides Him answer them not at all, except as he is answered who stretches forth his two hands toward water that it may reach his mouth, but it reaches it not. And the prayer of the disbelievers is but a thing wasted. (Al Quran 13:15)
Christian apologists want to make a case for Christianity based on laws of nature and science, by showing that there ought to be a Transcendent Creator of our universe.  They make this case, in one breath, and in the very next, deny all of science, by insisting on Eucharist, man-God of Jesus, who is not Transcendent, resurrection and miracles that violate laws of nature.
Atheists are right in exposing the irrationality of the Christian dogma. However, the Christians are right in as far as their claim that there needs to be a Creator of this universe, Who employed natural means to do His work. However, both parties in their self-conceit are not listening to how Islam resolves their conflict; Islam as understood by the Ahmadiyya Muslim Community.
In a trilateral discussion between atheists, Christians and Muslims, I believe we can appreciate reality better and come up with better theology, especially if the discussion not only involves the Creator and purpose of the universe, but, also His complete Transcendence, Original Sin and evolution of life on our planet, Trinity, Mother Mary and Eucharist.
No wonder, majority of the young generation, according to a recent poll in UK is agnostic or atheist.

Sigmund Freud remembered all his life the disgust and bitter disappointment he felt as a boy of ten years when hearing that his father refused to defend himself against the anti-Semite bullies who pushed him off the side walk of his home town, suggesting that a Jew should not walk on the side walks and leave it for the self righteous Nazis. This framed his spiritual struggle for the whole of his life that can be considered as a conflict between his Jewish identity possibly his faith and the anti-Semite Christian majority of the time.
His journey in psychoanalysis can be framed and understood by reviewing achievements and academic career of Jean-Martin Charcot. "Diseases can be caused by ideas" said Charcot and it heralded a new era in human understanding. This laid the foundation of Psychology, an identity separate from Neurology, which was rooted in physical causes. Charcot, a French neurologist was the founder (with Guillaume Duchenne) of modern neurology and one of France's greatest medical teachers and clinicians. He became a professor at the University of Paris (1860–93), where he began a lifelong association with the Salpêtrière Hospital; there, in 1882, he opened what was to become the greatest neurological clinic of the time in Europe. A teacher of extraordinary competence, he attracted students from all parts of the world.
In 1885 one of his students was Sigmund Freud, and it was Charcot's employment of hypnosis in an attempt to discover basis for hysteria that stimulated Freud's interest in the psychological origins of neurosis. Charcot was the "foremost neurologist of late nineteenth-century France" and has been called "the Napoleon of the neuroses." Allah says:
Those who believe, and whose hearts find comfort in the remembrance of Allah. Aye! it is in the remembrance of Allah that hearts can find comfort; 'Those who believe and do good works — happiness shall be theirs, and an excellent place of return. (Al Quran 13:29-30)
